"Season Kick-Off" is mandatory for all student-athletes interested in participating in
a Sport (incoming freshmen, and upper classroom) and should be present with a parent/guardian.


SPORTS PHYSICALS are being offered through the School-Based Health Center.

APPOINTMENTS: Parents can call or students should go to the Health Center to make an appointment for a physical.

No walk-ins will be accepted.

The School-Based Health Center will open the week 
                            of Aug. 12, 2019

To schedule and appointment for a physical exam
on or after 8/12/19 call 860-704-4590 to leave a message, and a  coordinator will return your call to schedule the appointment. 

**FALL Season Physicals: The physical date must be dated 
on or after November 14, 2018, in order to be accepted for clearance by the School Nurse. Sports Physicals must be active/not expire through the ENTIRE season of the sport being played.

Nov. 13th - Powderpuff ONLY
Nov. 19th - Girls Basketball, Ice Hockey, Wrestling
Nov. 20th - Boys Swim/Diving, Cheer, Unified
Nov. 21st - Boys Basketball
Nov. 25th - Boys/Girls Indoor Track
Dec. 3rd - Make-ups
Dec. 4th - Make-ups


* Impact testing is MANDATORY before participating in a sport and for Freshman, newcomers, transfers and those with expired baselines (good for 2 years). 

All forms below must be completed and turned in to your coach before students are eligible to practice/play any sport:

We highly recommend that all MHS Student-Athletes sign-up to receive services from the SBHC.
