In order to participate in any sport, you must do the following:
Register on Family ID
Have an updated Physical (information below, please reach out to your coach or our nurses if you have any questions with physicals)
Physicals have to be dated ON or AFTER TBD to be counted for the Spring 2026 season.
Get your sports physicals done at the MHS School-Based Health Center
To schedule an appointment, please call (860-704-4590). Remember to have the Pre-Participation Physical Examination Form and the School-Based Health Center (SBHC) - enrollment form (only if you are not a member) completely filled out before your appointment.
