2026 MHS Fall Sports: Sign Up Today & Get Your Physical Ready!

Hello MHS Students and Families,
Are you ready to hit the field, pool, or course this autumn? Fall sports are just around the corner, and registration is officially open!
Below is everything you need to know to get registered and cleared to play.
1. How to Join
- View Offerings: Click HERE for detailed information on our Fall sports offerings.
- Register Online: Register today on Family ID. Please note that FamilyID will close on August 31, 2026, at 11:00 PM.
- Stay Updated: Check the MHS Athletic Website for regular updates, or contact our Fall sports coaches and the athletic department with any questions.
2. Physical Expiration & Eligibility (Crucial)
To participate on the first day of practice or tryouts, your child must have an up-to-date physical on file with the nurse's office:- Eligible Date: Physicals must be dated on or after November 12, 2025, to count for the Fall 2026 season.
- Expiration Rule: If your child's physical expires between August 13, 2026, and December 12, 2026, they are NOT eligible to participate on day one.
- Please contact our school nurses if you have any questions about your child's physical status.
3. Need a Physical? Summer Appointments Available!
The MHS School-Based Health Center (SBHC) is offering sports physicals this summer between 8:00 AM and 2:00 PM on the following dates:- July: 14, 15, 21, 27, 28 & 29
- August: 3, 4, 5, 11, 12 & 13

- Before your appointment, you must completely fill out these forms:
- Pre-Participation Physical Examination Form
- School-Based Health Center Enrollment Form (only required if you are not already a member)
